New support for low-income Californians
Starting January 1, 2025, California introduced two major policy changes to ease Medicare costs for low-income residents:
- The state converted to a Medicare Part A “buy-in” model, so eligible individuals who aren’t already premium-free for Part A have their Part A premium covered. Justice in Aging
- People receiving Supplemental Security Income (SSI) will be automatically enrolled into the Qualified Medicare Beneficiary (QMB) program, which helps pay premiums, co-pays, and deductibles. Justice in Aging
Why this matters: If you’re eligible for both Medicare and Medi-Cal (dual eligible), or you’re low income and on Medicare, these new policies can significantly reduce your out-of-pocket costs.

Expanded “Medi-Medi” integrated plans across more counties
As of the annual enrollment period beginning October 15, 2025, the state’s Medi‑Medi Plan program (for people who have both Medicare and Medi-Cal) is expanding into 29 additional California counties. DHCS+1
Details:
- These plans combine Medicare and Medi-Cal benefits under a single health plan card, with one care team and unified services. DHCS
- The expansion brings the total participating counties to 41. DHCS
- Eligible enrollment period: Oct 15 to Dec 7, 2025 — coverage starts Jan 1, 2026. DHCS
Why this matters: For dual-eligible members, this means simpler coverage, fewer separate cards and phone numbers, and more coordinated care. It’s especially relevant in counties such as San Luis Obispo, Santa Barbara and others newly added.
